Abstract
Introduction: A detailed understanding of root canal morphology is required to achieve good endodontic treatment. Additional roots and canals present on a tooth will require special attention to prevent failure of the treatment. The clinician might not be familiar with the anatomical variations. In most cases, radix entomolaris have smaller and severe curvature which complicates the endodontic treatment.
Case report: A 22-year-old female presents with a chief complain of severe pain when chewing in the lower right posterior tooth. Clinical examination showed occlusal cavity on the right mandibular first molar. Radiographic image revealed an additional distal root on lingual with radiolucency on the periapical.
Discussion: The presence of additional roots becomes one of the challenges because clinicians might miss one or more canals. The radiographs should be taken in different projections. Careful reading of angled radiographs and assessing distinguished anatomical landmarks can be aided in detecting additional roots.
Conclusion: In this report will be discussed the identification of additional roots and canals during endodontic therapy which is fundamental for a long-term success of the treatment.
