Abstract
Objective: This study aimed to evaluate the dental service performance of Indonesian primary healthcare facilities (PHCFs) operating under the capitation payment system
Material and Method: This analytical study utilized secondary data from the social security administering body (BPJS). Total sample included 47,847PHCFs consisting of community health centers (CHCs), clinics, and solo practice dentists (SPDs) partnering with BPJS to provide dental services in 2019, 2021, and 2022. The independent variable was the PHCF type, whereas the dependent variables were the performance of dental services as measured by the visit and referral ratios. A one-way analysis of variance test was used to compare means across groups.
Results: A low national average visit ratio was 0.63%, with SPDs having the highest (1.46%), followed by clinics (0.95%) and CHCs (0.29%). This study found that the national average referral ratio was 19.24%. Clinics had the highest referral ratio (24.15%), followed by CHCs (17.02%) and SPDs (8.83%), with SPD referrals declining in 2022 than in 2019. Findings reveal significant differences in visit and referral ratios across PHCF types (p < 0.05), indicating that varying capitation mechanisms influence service utilization and cost control.
Conclusion: SPDs demonstrated the best performance, emphasizing the need to refine capitation models for improved efficiency.
