Abstract

ABSTRACT

Background: The pulp has a defense system against inflammation involving the immune system. When the surface becomes inflamed, leukocyte accumulation occurs followed by proinflammatory cytokines released. Neutrophils, the largest part of leukocytes, work surround infected areas, and phagocytize pathogens. Tumor Necrosis Factor-alpha (TNF-α), the main cytokine proinflammatory agent, provides a rapid response to infection. Application of calcium hydroxide (CH) in reversible pulpitis releases hydroxyl groups with an alkaline pH and has the potential effect to pulpal irritation and necrosis. So that a combination of medication was developed to reduce the inflammatory effect of CH. Several studies reported that haruan fish contains high levels of albumin has functions as an anti-inflammatory and stimulates tissue repair, by maintaining osmotic pressure and microvascular integrity.

Objective: To evaluate the combination of haruan fish extract and CH on reducing the number of neutrophil cells and TNF-α at concentrations of 25%, 50% and 75%.

Methods: Post test in vitro laboratory experimental study with a control group design. A total of 24 rat tooth were divided into 4 groups: the control group (100% CH), the treatment group used a combination of haruan fish extract and 25%, 50% and 75% CH, then evaluated on the 1st, 3rd and 5th day, and then observed under the microscope.

Results: The mean of the number of neutrophil cells and TNF-α decreased significantly on observations on days 1st, 3rdand 5th in the 25% and 50% haruan fish extract combination group but the decreased number between the 50% and 75% haruan fish extract combination group is not significantly different.

Conclusion: The combination of 75% haruan fish extract and 25% CH has been shown to be effective in reducing the number of neutrophil cells and TNF-α on the 5th day of observation.
