Abstract

Objective

This case report aimed to describe the radiographic features of a maxillary unicystic ameloblastoma in a tooth-bearing region extending to the maxillary sinus that has undergone inadequate root canal treatment and masquerading as an inflammatory odontogenic cyst.

Methods

A 42-year-old-female underwent a Cone Beam Computed Tomography (CBCT)-3D examination. The patient had previously undergone orthopantomography. Extraoral examination showed asymmetrical face with intraoral swelling of the left maxilla measuring approximately  3x1x1 cm and firm in consistency. There was carious pulp of tooth 25.

Results

Orthopantomograph (OPG) showed a sizeable well-defined corticated unilocular radiolucent lesion that extending from alveolar to the left maxillary sinus cavity; masquerading as a large odontogenic cyst initially. From CBCT-3D examination we found that the lesion  was associated with a left upper premolar tooth that had a history of underfilling root canal treatment. Apparently, the lesion was histologically diagnosed as unicystic ameloblastoma.

Conclusion

A large unicystic ameloblastoma in the maxilla extending to the maxillary sinus is rare and may mislead the suspect of radiodiagnosis as seen in this present case. I this case, we can define the diagnosis through histological examinations, which have become the gold standar.
