Abstract
Background:melanotic neuroectodermal of infancy (MNTI) is a very rare benign pigmented neoplasm originating from the neural crest associated with increased excretion of vanillylmandelic acid. Fewer than 500 cases have been reported to date. Usually painless, often based on an epithelial surface, blue to black in color with extensive and aggressive growth. MNTI vascularization is rarely reported, requiring MRI radiographic examination, angiography, embolization and biopsy to support diagnosis and surgical management as well as minimizing complications.
Case report: a 2 year old male brought by his parents to the Oral Surgery clinic at Dr. Sardjito Hospital complained that the lump on the front of his upper jaw had gotten bigger in the last few months. Patients are often fussy and have difficulty eating and drinking. Extraoral examination: there is facial asymmetry with an intraoral tumor of the anterior maxilla pressing on the lips and nose. Intraoral examination: there is a tumor in the anterior maxilla, right deciduous canine region to left deciduous molar, round shape with a diameter of ± 9cm with a rough bluish brown surface, pressing against the alar base, hard, without a stem, painful tenderness, does not bleed easily. MRI examination, angiography and embolization were carried out to confirm the diagnosis and minimize complications and maximize results.
Conclusion: Management of MNTI cases is maxillary resection with preoperative procedures including angiography and embolization, providing maturity in planning before action is carried out. This procedure can minimize complications during the duration of the operation and provide visualization for the operator in removing the tumor as a whole and providing maximum results without recurrence.
